Telehealth

We provide Physical Therapy & Performance Training telehealth options for out-of-town patients and clients or those who want to receive care in the comfort of their home.

Out of Network

To deliver better quality of care without being restricted by the rules and restriction of insurance, we are an out-of-network practice.

We Accept Insurance

Payment in full is required at the time of your appointment. We can bill your insurance provider and if any money is owed to you, your insurance provider will reimburse you the contracted amount or percentage. If you have questions about how reimbursement works from your insurance provider, we suggest contacting your insurance provider prior to your MAVEN appointment.

Direct Access

Under California Law AB1000, you do not need a prescription to receive physical therapy evaluation and treatment services for a period of up to 45 calendar days or 12 visits.

What are your COVID-19 policies?
We follow all CDC guidelines, and we are committed to a safe and clean environment. All employees are vaccinated, and all employees and clients are required to wear a mask indoors.
What is Direct Access in the State of California?
As of 2014, California law under AB1000 allows licensed physical therapists to provide direct physical therapy evaluation and treatment services for a period of up to 45 calendar days or 12 visits, whichever occurs first. This means patients can be evaluated and treated by a physical therapist without needing a practitioner or other healthcare provider's referral. 

In other words, Direct Access lets you visit a physical therapist when you feel you need to see one. Sometimes you may have back or knee pain because of an obvious injury like a sprain or a pulled muscle—direct access gives you the power to decide if you need physical therapy.
Do I need to do anything in advance?
You will be sent an intake form via email before your first visit. Please fill out the intake form before you arrive, as it will save you time and allow us to start your session right away.
Is there Parking?
There is free limited parking in the back of the building. Please enter from Wilshire Blvd. down the alley between 7th and Lincoln. Additionally, there is metered parking nearby.
What should I wear?
Please bring comfortable workout clothes and, if injured, make sure the area can be exposed for assessment.
What should I bring?
Please bring your insurance card, prescription (if you have one), and any diagnostic documents or images (if you have them).
Is there a place to change?
In addition to restrooms, we have private rooms available for you to change clothes.
What is the cancellation policy?
If you need to cancel your appointment, please let us know 24 hours in advance. Cancellations within 24 hours will be subject to a late fee. If you need to cancel an appointment on Monday, you must inform us by Saturday at 1:00 p.m.
Do you offer packages?
Yes! We offer a variety of packages for physical therapy, performance training, and Pilates. Please call the concierge for pricing information.
